Miriam Picchi began her football career with Tecchiena before moving to Lazio for the 2011–2012 season, where she made her Serie A debut in 2012. After four seasons with Lazio—two in Serie A and two in Serie B—she joined Res Roma in 2015, where she played until 2018. She then returned to Lazio for the 2018–2019 Serie B season.In 2019, she transferred to Ravenna, where she spent two seasons before joining Como Women in July 2021, helping the team earn promotion to Serie A.
During the 2024–2026 season, she was honored by the AIC (Italian Footballers' Association) for scoring the best goal of the 2023–2024 season. On March 15th, 2026 in the match vs Ternana, She reached 150 caps in Serie A.
